Looking for someone with excellent customer service skills. Strong interpersonal & written communication skills. Ability to multi-task, work independently, & work efficiently in a fast-paced environment with changing priorities.**
Common types of patients & job duties: Dealing with all payers, uninsured, homeless. Completing initial assessments, DC planning (including nursing homes, home health, DME, prior authorizations, etc) Case load/# patients per day/consults per day Caseloads can range from 15-28 on average EMR: Cerner Is this position for UR, discharge planning, or both? Discharge planning What areas do they cover (Inpatient, Outpatient, ER, Home Care, etc.): Inpatient Experience or 'Must have' skills: 3 years' nursing experience in an acute hospital setting- Prior care coordination experience in a CommonSpirit/Dignity/CHI hospital Call Requirement: None Scrub Color/Dress: Business casual & lab coat
