PROGRAM ASSISTANT/CNA
Responsible to: Adult Day Care RN
JOB SUMMARY:
Provides personal care and assistance with activities of daily living to Adult Day Care Program registrants. Organizes and implements an on-going program of activities designed to meet the physical, mental and psychosocial well-being of each registrant. This position requires working a flexible schedule that accommodates the needs of registrants.
RESPONSIBILITIES:
· Performs personal care functions and other restorative nursing measures for registrants according to the registrant’s care plan.
· Assists with all activities of daily living as needed (eg. - bathing, dressing, toileting, shaving) to maintain the registrant’s highest functioning level.
· Dispose of soiled linen per facility policy.
· Reports changes in registrant’s conditions to the Program RN and Coordinator and/or LPN as necessary.
· Serves lunch and applicable nourishments/refreshments.
· Assists with feeding as indicated in registrant’s care plan.
· Documents appropriate charts, reports and records as needed pursuant to established procedures and practices.
· Attends educational in-services at least three (3) hours every quarter to meet the annual requirement of twelve (12) hours per calendar year.
· Ensure that department work areas are maintained in a clean, sanitary, and safe manner.
· Use supplies in an effective, efficient manner.
· Encourages registrants to participate in activities programming and transportation and events held off-site.
· Leads small group and individual activities.
· Works with each registrant to ascertain individual leisure time, activities, hobbies and interests.
· Participates in department meetings and staff development programs.
· Maintains confidentiality of all registrant’s information.
· Performs other duties as assigned.
· Actively participates in Customer Service Initiatives and attends all mandatory facility in-service educational trainings.
· Is aware of, signed, and received a copy of the document outlining the expectations of all CNAs.
· Maintains and safeguards entrusted confidential information, follows the requirements and regulations of HIPAA and PRNC’s Compliance Plan and it’s Code of Conduct.
QUALIFICATIONS:
· If you are not a credentialed CNA, you have (60) days to obtain your certification through our training program
· Current certification as a New York State Nurses Aide in good standing on the NYS Nurse Aide Registry.
· Knowledge of and interest in structured activity programming including arts and crafts.
· Experience working with senior population.
· Ability to prioritize multiple tasks.
· Familiarity with healthcare documentation.
· Ability to lead small groups and individual activities.
